Registration Success!

Your registration form has been submitted successfully – thank you. Please check your email for a confirmation message. If you do not receive your confirmation email, please email missellen@monmouthacademyofballet.com.

There are two additional steps to complete before your first class:

  1. Prior to the student’s first class, every student (or legal parent or guardian) must agree to and sign the Monmouth Academy of Ballet’s Release of Liability. It is available here: https://monmouthacademyofballet.com/resources/documents/ This document must be executed in-person.
  2. Prior to the student’s first class, every student’s payment arrangements must be finalized. You have the option of paying by credit card or by cash or check. For either scenario, we require that we keep a credit card on file. For your security, we will collect your credit card information either over the phone or in person.

*Reminder, you have agreed to our payment terms which are as follows:
All fees must be paid when due, and there will be no refunds for any portion of tuition and fees paid. If paying by credit card, you hereby authorize Monmouth Academy of Ballet to charge your card when payments are due. If you elect to pay by cash or check, and your payment is more than 10 days past due, you hereby authorize Monmouth Academy of Ballet to charge your card for the amount due, plus a $50 fee. If your check is returned for any reason, you hereby authorize Monmouth Academy of Ballet to charge your card for the amount due, plus a $50 fee.
